A. Application for New Service Connection
Schedule of Availability of Service
Monday to Friday, except holidays (8 AM to 5 PM, no noon break)
Who may avail of the service?
Any person within the area of responsibility of Polanco Water District without an existing service connection
What are the Requirements?
- Proof of ownership of land, which can be any of the following:
- Land title (photocopy)
- Building permit (photocopy)
- Certificate of residency from barangay
- Electric bill
- Deed of sale (photocopy)
- Photocopy of 1 Valid ID or recent Community Tax Certificate
- Lease of Contract Agreement (photocopy)
- Installation fee (PHP4,000)
Typical Duration:
- For installation: Maximum of 2 days, provided the applicant already has in-house piping
- For application: 10 minutes

B. Application for Transfer of Service Connection

Any person within the area of responsibility of Polanco Water District with an existing service connection
What are the Requirements?
- Proof of ownership of land, which can be any of the following:

Any person or representative who wishes to pay for goods and services rendered or about to be rendered
What are the Requirements?
Any one of the following:
- Billing Notice
- Statement of Account
- Account Number
- Complete Account Name
- Application Form
Typical Duration: 2 minutes per payor

Name of account holder or account number

Any of the following:
- Billing Notice
- Statement of Account

- Pay all outstanding obligations – reconnection fee (P225)
- Reconnection – next day
